The process brings together information gathering, individual formulation, agreed goals and regular review — so support is thoughtful, transparent and connected to realistic next steps.
01 — Referral
A school, local authority or relevant professional submits a referral or makes an initial enquiry.
02 — Information Gathering
Information is gathered from professionals involved and, where appropriate, the young person and parent or carer.
03 — Initial Assessment
We explore current difficulties, strengths, risks, relationships, educational history, interests, needs and barriers to engagement.
04 — Psychological Formulation
Available information is brought together to understand what may be contributing to difficulty and to guide intervention.
05 — Individual Support Plan
Meaningful, realistic and measurable goals are agreed around the young person’s needs, interests and strengths.
06 — Intervention
Bespoke mentoring and psychologically informed support is delivered through conversation, practical work, activities or educational engagement.
07 — Monitoring
Progress is monitored against agreed outcomes with communication across the relevant professional network.
08 — Review & Next Steps
We review whether support remains appropriate and identify sustainable next steps, including reintegration or progression.
